Eficacia clínica de la terapia de reemplazo hormonal en pacientes con síntomas climatéricos

El objetivo fue determinar la eficacia clínica de la terapia de reemplazo hormonal (TRH) en pacientes con síntomas climatéricos. Se incluyeron 532 pacientes atendidas en la consulta uroginecológica durante el período comprendido entre abril de 2023 y noviembre de 2025, clasificadas según el diagnóstico en insuficiencia ovárica prematura (n = 72), menopausia temprana (n = 98) y menopausia tardía (n = 362). A todas las participantes se les realizó historia clínica, examen físico y pruebas de laboratorio, de acuerdo con las recomendaciones establecidas en guías internacionales. Previo al inicio del tratamiento, se proporcionó información sobre los beneficios y riesgos asociados con la TRH. La respuesta clínica se evaluó a los 2, 6 y 12 meses mediante una escala analógica visual de 0 a 10 puntos, donde 0 correspondió a ausencia de mejoría y 10 a mejoría total. Se consideró falta de respuesta terapéutica en las pacientes que no reportaron mejoría, incluidas aquellas que no iniciaron o interrumpieron el tratamiento debido a temor al uso del medicamento, dificultades para acceder al tratamiento o comprensión insuficiente de la información proporcionada. Al establecer como punto de corte una mejoría clínica ≥70 %, se observó una eficacia del tratamiento del 85,0 % (IC 95 %: 0,82–0,88; p < 0,001); mientras que, con un punto de corte ≥80 %, la eficacia fue del 76,9 % (IC 95 %: 0,73–0,80; p < 0,001). No se observaron diferencias estadísticamente significativas en la eficacia entre los grupos de tratamiento (p = 0,176). Se concluye que la terapia de reemplazo hormonal mostró una elevada eficacia clínica en pacientes con síntomas climatéricos; no obstante, es necesario fortalecer la información proporcionada sobre sus beneficios y riesgos, así como mejorar el acceso a los métodos diagnósticos y terapéuticos y garantizar un seguimiento multidisciplinario que favorezca la adherencia y los resultados clínicos.
